SACRAMENTO, Oct. 26, 2026 — A federal eligibility change took effect October 1 for some noncitizens who receive Medi-Cal, but California health officials say the change does not mean every affected member immediately loses full-scope coverage. California is using state funding to preserve that coverage through July 1, 2027, for the affected group described in state guidance.
What changed on October 1?
Under federal H.R. 1, some individuals previously considered qualified noncitizens no longer meet the federal funding definition for full-scope Medicaid. The California Department of Health Care Services (DHCS) says affected members will be automatically transitioned to state-funded full-scope Medi-Cal. This is a change in the financing and eligibility framework, not an instruction for everyone to file a new application.
Does an affected member need to do anything now?
According to DHCS’s October 2026 notice, affected members do not need to take action solely because of this transition. DHCS began mailing notices in September. Members should contact their county Medi-Cal office if their immigration status has changed or their information needs updating. Other ordinary reporting and renewal obligations may still apply to the individual.
What about 2027?
Separate federal and state implementation changes are scheduled for 2027, including requirements affecting certain adult eligibility groups. The details differ by eligibility category; timelines should not be collapsed into a single deadline. California is establishing programs to help members understand renewal cycles and coverage changes. An eligibility notice, rather than a generic online post, controls the next step in an individual case.
How to check the correct information
Read official DHCS communications, keep the most recent notices from your county office, and use the contact details on those notices to correct inaccurate records. People who do not receive notices should not assume they fall into the affected category. Eligibility may depend on age, immigration status, income, household circumstances and specific program rules.
For the wider community, local access to health care remains a major concern throughout California’s rural areas. Readers can find neighborhood reporting through California58 local newspapers.
